This guidance emphasizes the need for stronger country-led research and development (R&D) frameworks, addressing the persistent global inequalities in clinical trials, particularly between high-income countries (HICs) and low- and middle-income countries (LMICs). The document provides specific recommendations for national and regulatory authorities, as well as funders, to enhance trial design, participant diversity, and infrastructure. Key objectives include ensuring faster and more equitable access to safe and effective health interventions and embedding clinical trials into routine healthcare services.
