Nipah virus continues to cause recurrent outbreaks in Bangladesh and India. With no approved vaccines or treatments currently available, prevention relies heavily on reducing opportunities for spillover, detecting cases early, and responding quickly when outbreaks occur.
This recent article argues that communities should not simply be recipients of public health information, but active partners in Nipah preparedness and response. We explore four levels of community engagement:
 
Community-oriented: Design prevention strategies around local beliefs, livelihoods and practices. For example, approaches to reducing risks associated with raw date palm sap should focus on locally acceptable ways of reducing exposure rather than simply prohibiting established practices.
Community-based: Work through trusted community structures and frontline workers to strengthen awareness, surveillance and early detection. Community health workers and local health teams can help identify suspected cases, facilitate referrals and contact tracing, and address misinformation.
Community-managed: Enable local stakeholders, including religious leaders, teachers, youth and women's groups, community volunteers and local government representatives to take a greater role in organising preparedness activities, communicating risks and supporting affected households during outbreaks.
Community-owned: Support communities to shape and lead locally appropriate prevention approaches. This could include identifying practical risk-reduction measures and participating directly in decisions about how interventions are designed and implemented.
 
Moving along this spectrum means gradually shifting communities from being recipients of information towards becoming partners in decision-making and preparedness. While evidence specific to Nipah remains limited, learnings from other outbreaks has proved time and agian that locally grounded community engagement could strengthen trust, early detection, outbreak response and preparedness for future vaccines and therapeutics.
